1. IF the highest value EVER recorded for Total Cholesterol for a patient on or before the specified date is greater than:
a) serum: 5 mmol/L
OR
b) serum: 193.35 mg/dL
OR
c) plasma: 4.8544 mmol/L
OR
d) plasma: 187.7184 mg/dL
