IF the highest value EVER recorded for Triglycerides for a patient on or before the specified date is greater than: a) serum: 1.7 mmol/L OR b) serum: 150.569 mg/dL OR c) plasma: 1.6521 mmol/L OR d) plasma: 146.3256 mg/dL
